APPLICATION FOR ADMISSON PROCESS
Please follow the process below if you would like for your child to be considered for enrollment at Sunrise Academy.
Step 1
Go to www.sunriseacademy.net
Choose Forms
Choose Admission Applications
Print Information. (This includes the Application for Admission, Required Documents and Admission Policy.)
Step 2
Complete Application for Admission and mail it with the required documents and a $25 non-refundable processing fee to:

Applications will not be accepted at the school. You must mail all documents to the address above.
For EdChoice COLUMBUS CITY SCHOOL Voucher applicants – You will be called to Sunrise Academy to complete further paperwork if your child is accepted for enrollment and qualifies for a school voucher. NOTE: Students will not be considered for admission if the application is incomplete and/or other required documents are missing.
ALL APPLICATIONS MUST BE POSTMARKED BY MARCH 31st.
