2010-2011 Free and Reduced Breakfast-Lunch Family Application
Below is the 2010-2011 Free and Reduced Breakfast/Lunch Family Application. This application needs to be renewed every academic school year. Please fill out the application completely and return to Sunrise Academy no later than August 6th (one application per family). If you receive Food Stamps, you will need to include your 10 Digit Food Stamp Case Number. If you do not receive Food Stamps, you will need to fill out section 4 on the application.
