If you would like to apply for school bus transportation (new students or address changes only), download the appropriate form below. If you live in the Columbus Public School District, please fill out the Columbus Bus form and return it to Sunrise Academy. If you live in the Hilliard CitySchool District, please fill out the Hilliard Bus form and return it to the address listed on the form. Dublin City School residents must call Dublin Transportation at (614) 764-5926.
