2010-2011 Application for Admissions

Please follow the process below if you would like for your child to be considered for enrollment at Sunrise Academy.

All applications must be postmarked by March 31st.

Free and Reduced Price School Meals Family Application

The 2009-2010 Free and Reduced Price School Meals Family Application is now available. This application needs to be renewed every academic school year. Please fill out the application completely and return to Sunrise Academy (deadline 8/3/09). If you receive Food Stamps, you will need to include your 10 digit Food Stamp Case Number. If you do not receive Food Stamps, you will need to fill out section 4 on the application. Latchkey Program

If you are interested in latchkey for after school care, please fill out the latchkey application and turn it into the school office. Latchkey Application

Sunrise Academy Fees

Application Fee (New Students Only)	$25 per application payable on submission of application
Material Fee	$100 per student; Must be paid by the first day of school.
Pre-Kindergarten Tuition Part-Time	$3,500 annual; Must be made through automatic monthly withdrawal.
Pre-Kindergarten Tuition Full-Time	$6,000 annual; Must be made through automatic monthly withdrawal.
Kindergarten Tuition	$4,500 annual; Must be made through automatic monthly withdrawal.
1st Grade - 8th Grade Tuition	$4,375 annual; Must be made through automatic monthly withdrawal.

Complete Application for Admission and mail it with the required documents and a $25 non-refundable processing fee to:

Applications will not be accepted at the school. You must mail all documents to the address above.

For EdChoice COLUMBUS CITY SCHOOL Voucher applicants – You will be called to Sunrise Academy to complete further paperwork if your child is accepted for enrollment and qualifies for a school voucher. NOTE: Students will not be considered for admission if the application is incomplete and/or other required documents are missing.
