Community Event Consent
By signing this consent, I, the undersigned, hereby acknowledge and agree to the terms and conditions outlined below for my participation in the community event organized by Sunrise Academy with the assistance of the organizing entity, its affiliates, employees, or volunteers.
Terms and Conditions:
Assumption of Risk: I understand and acknowledge that participation in the community event may involve certain risks and hazards, including but not limited to physical injury, property damage, and exposure to communicable diseases. I voluntarily assume all such risks and agree that neither Sunrise Academy nor the organizing entity, its affiliates, employees, or volunteers shall be liable for any such risks.
Waiver of Liability: I hereby waive, release, and discharge Sunrise Academy, the organizing entity, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained by me during the community event, including but not limited to those caused by the negligence or fault of Sunrise Academy or the organizing entity.
Indemnification: I agree to indemnify and hold harmless Sunrise Academy, the organizing entity, its directors, officers, employees, agents, and volunteers from any and all liabilities or expenses (including reasonable attorneys' fees) arising from or related to my participation in the community event, including but not limited to any claims made by third parties.
Compliance with Rules and Regulations: I agree to comply with all rules, regulations, and instructions provided by Sunrise Academy and the organizing entity during the community event. Failure to comply may result in my expulsion from the event without a refund or compensation.
Medical Authorization: In the event of a medical emergency, I authorize Sunrise Academy and the organizing entity to seek medical treatment on my behalf. I understand that Sunrise Academy and the organizing entity will make reasonable efforts to contact me or my emergency contact before seeking medical treatment, but they reserve the right to act in my best interest if unable to reach me.
Consent: By selecting the options, I acknowledge that I have read and understand the terms and conditions outlined in this consent form. I voluntarily agree to participate in the community event organized by Sunrise Academy with the assistance of Sunrise staff and volunteers and hereby release Sunrise Academy, the organizing entity, its affiliates, employees, and volunteers from any liability arising from my participation.
