FIELD TRIP EVENT CONSENT AND LIABILITY WAIVER
Please read carefully to indicate your agreement
In consideration of my child's participation in the above-referenced field trip or event organized by Sunrise Academy, I, the undersigned parent/guardian, hereby agree to the following terms and conditions:
Consent for Emergency Medical Treatment: In the event of an accident or emergency during the field trip, event I authorize the designated representatives of Sunrise Academy to obtain necessary medical/hospital care for my child, including transportation to a medical facility if deemed necessary. I understand that every effort will be made to contact me or the emergency contacts listed below before any medical treatment is administered.
Choice of Physician: I authorize the designated representatives of Sunrise Academy to select and engage medical professionals to provide care and treatment to my child as deemed necessary. I understand that if the physician designated by me is unavailable, any licensed physician or surgeon may provide care.
Responsibility for Costs: THE UNDERSIGNED PARENT/GUARDIAN FULLY UNDERSTANDS THEY ARE RESPONSIBLE TO PAY ALL COSTS INCURRED AS A RESULT OF THE FOREGOING. If your child is injured on a field trip, or event.
Waiver of Claims: I acknowledge that participation in the field trip, event involves certain risks, including but not limited to accidents, injuries, and illness. As a condition of my child's participation, I hereby waive and release any and all claims, demands, causes of action, and liabilities against Sunrise Academy, its employees, volunteers, agents, representatives, and governing board arising out of or related to my child's participation in the field trip, event.
Indemnification: I agree to indemnify and hold harmless Sunrise Academy, its employees, volunteers, agents, representatives, and governing board from and against any and all liabilities, damages, losses, and expenses, including reasonable attorney's fees, arising out of or resulting from my child's participation in the field trip, event.
